Passport Application and Renewal

PASSPORT REQUIREMENTS FOR FIRST TIME APPLICANTS:

1. Birth certificate (Authenticated issued by NSO) Original/Photocopied

2. Baptismal Certificate (Original/Photocopied)

3. For Minors (Below 18 years old)

a. Personal appearance of mother and child

b. If minor is not traveling with either parent, submit an original and photocopy of DSWD Clearance and Affidavit of Support and Consent indicating the name of the traveling companion and relationship to the minor.

c. If both parents are abroad, such an affidavit must be authenticated by the nearest Philippine Embassy or Consulate General.

d. If both parents are abroad, person applying in behalf of the minor musty submit a Special Power of Attorney authenticated by the nearest Philippine Embassy and Consulate General.

e. Passport and photocopy of the passport of person traveling with the minor.

(If the minor is 8 years & below and traveling unaccompanied by mother, mother’s consent is required)

4. Marriage Contract (for women only) Original/Photocopied.

5. School Ids (Original/Photocopied)

6. 4 pcs. Passport size pictures(Royal Blue background) (Applicants must wear dark colors with collar)

7. NBI Clearance

RENEWAL OF PASSPORT:

1. Old Passport (Original/Photocopied)

2. 4 pcs. Passport Size (Royal Blue background) (Applicants must wear dark color with collar)

PHP 1,300 – 15 working days

PHP 1,500 – 10 working days
